I agree with your view of why BI projects fails. Nevertheless I have my own point of view about that too. I think companies fail to leverage good BI Projects because they tried to implement a certain level of BI that is not supported by their actual level of BI processes. This could be avoided by knowing before hand your maturity level of BI and then drawing the respective roadmap to get to the level of BI the company wants. This roadmap would contains a set of projects that implemented in batch (or in parallel) would take the BI culture to another level.

Well, I hope I was clear enough on my point.
